VHM and M-Series Service Bulletin
The purpose of this bulletin is to inform you of a field action GCX is taking to address a possible safety issue. GCX recently received a complaint that a patient module housing suspended from the underside of a wall mount arm became detached. After repeated cycling, the swivel mechanism loosened due to a manufacturing error. This was one failure out of approximately 40,000 similar arms sold between February 1, 2008 and September 24, 2008. While it is extremely unlikely that you would experience an issue, GCX is taking a proactive approach to eliminate any risk. Note that there is only a risk of failure in applications where a load, such as a patient module housing, is mounted to the underside of the arm.

GCX is providing you retention cable kits at no charge so that arms installed over this period may be retrofitted, and safety of the arm assembly thereby ensured. This cable does not interfere with normal operation or cleaning of the support arm. In most cases, it will also not be visible to the user. Assembly to the arm can be completed in a few seconds. No tools or disassembly of the arm are necessary, and the installation will not interrupt the function of the support arm or mounted device.
